Family Law cases involving matters such as divorce, equitable distribution of property, child custody, child support, alimony, and spousal support are heard in Civil District Court. As a service to the public, the Clerk’s Office provides a sample form that may assist pro se litigants obtain an uncontested simple divorce. Unfortunately, due to the complexity of the other domestic issues, our office is not able to offer forms to assist you as a pro se litigant. Thus, if you have related claims for equitable distribution, child custody, child support, alimony, or spousal support, they are NOT covered by our sample form. 

The Clerk’s Office or anyone else contributing to the production or dissemination of these forms, instructions, or guidelines are not liable for any direct or consequential damages resulting from your use of the forms or the information provided. We CANNOT help you fill out the form or revise it to fit the circumstances of your case. Use these forms at your own risk. Any desired outcome from the use of these forms cannot be predicted or guaranteed.
