In order to receive transportation services from the Dare County Transportation System, passengers are requested to complete a New Rider Information Form and provide the following information:

• Name
• Address (physical and mailing)
• Telephone Number
• Date of Birth
• Other information may be requested if eligibility guidelines apply
• Type of disability (if applicable)
• Mobility aids (if applicable)

Note: The Dare County Transportation System must be kept updated on any address or telephone number changes.
